CareSource SNP is a plan for those individuals who are entitled to Medicare Part A, are enrolled in Medicare Part B, and also have Medicaid. With CareSource SNP you choose your primary care provider from the network of local primary care practitioners and have all of your care coordinated through your own physician. There is no copay for office visits to your primary care provider and referred specialists.

You will have no, or very small, prescription drug costs (depending on eligibility), including the availability of home delivery. Rest assured that you will have the best medical care available through the CareSource SNP.
