This Provider Directory lists the professionals, facilities and pharmacies that are available for your health care through the Mid Rogue Health Plan.

Provider Directory (updated 8/24/2010)

CareSource has contracts with pharmacies that equal or exceed CMS requirements for pharmacy access in your area.  There are a total of 71 network pharmacies (in our CareSource Service Area), 8 chain pharmacies (found both in and out of our Service Area), 5 Infusion Pharmacies, 5 Long-Term Care Pharmacies, and 14 Mail Order Pharmacies.

Primary Care Physicians

The Plan contracts with over 280 Primary Care Providers (PCPs) in Josephine, Jackson and Curry Counties. Each CareSource (except our Diamond PPO) member needs to select a PCP from this group of providers.

Specialty Care

The Plan contracts with over 480 medical specialists from Mid Rogue Independent Physician Association in Josephine county, PrimeCare in Jackson county, as well as independently contracted specialists in Curry County.

Hospitals

CareSource/Mid Rogue Health Plan has contracted with a network of hospitals to provide inpatient care and hospital-based outpatient diagnostic and treatment services for CareSource members.

Allied health professionals

The Plan contracts with a number of Allied health professionals to provide skilled nursing care, rehabilitation services, diagnostic services, and complementary medicine (chiropractors and acupuncturists).

Our service area encompasses all of Josephine, Jackson, Curry counties and Glendale and Azalea in Douglas County.
